The Stanford Chronic Disease Self-management Program, run by Western Sydney Local Health District, is for people in the local community who want to learn:

  • How to make better choices for their health and lifestyle
  • Gain a better understanding of their chronic disease.

This is a FREE 6-week program offered in a number of languages and held in local community venues. Morning tea is provided.

Who can attend

The program is for anyone over the age of 16 who have one or more of the following chronic diseases:

  • Diabetes
  • Heart failure
  • Heart disease
  • High blood pressure
  • Chronic obstructive pulmonary disease (lung disease).
